About This Protocol

Tirzepatide is a dual GIP and GLP-1 receptor agonist — a compound that activates both the glucose-dependent insulinotropic polypeptide receptor and the glucagon-like peptide-1 receptor simultaneously. This dual mechanism distinguishes it from single-receptor GLP-1 agonists. This protocol is compounded with Cyanocobalamin (B-12) to support energy metabolism.

This is a physician-supervised compounded medication available exclusively through licensed TeleWellnessMD® medical providers. The physician evaluates each patient individually. Not all patients will qualify.

Dual Receptor Mechanism
GIP receptors are expressed in the pancreas, adipose tissue, and bone. When activated alongside GLP-1 receptors, they produce complementary effects on insulin secretion, glucagon suppression, and gastric motility. According to clinical research published in the New England Journal of Medicine (2022), dual agonism has been studied in the context of metabolic function.
GLP-1 Receptor Component
GLP-1 receptors are expressed in the pancreas, brain, and gastrointestinal tract. When activated, they stimulate glucose-dependent insulin secretion and slow gastric emptying — affecting signals that regulate appetite and satiety.
Progressive Dosing
Protocols using dual receptor agonists typically begin at lower doses and titrate upward monthly, allowing physiological adaptation and reducing GI side effects. The physician determines the appropriate starting dose based on individual assessment.
Cyanocobalamin (B-12)
Vitamin B-12 is an essential cofactor in DNA synthesis and cellular energy metabolism. Its inclusion supports general metabolic function during the protocol and may help address common B-12 deficiencies in the population.
