About This Protocol

Tadalafil is a PDE5 inhibitor with a significantly longer half-life than sildenafil — approximately 17.5 hours versus 4-5 hours. This extended duration allows for two dosing strategies: low-dose daily use (5mg) for continuous effect, or higher-dose as-needed use (20mg) for a longer window of activity.

PDE5 Mechanism
Like sildenafil, tadalafil inhibits phosphodiesterase type 5, preventing cGMP breakdown in vascular smooth muscle. The resulting vasodilation in response to nitric oxide signaling supports vascular function.
Troche Form
The troche (lozenge) is dissolved in the mouth for buccal absorption. This bypasses first-pass hepatic metabolism, potentially achieving faster onset and higher bioavailability than tablet forms.
