About This Protocol

Sildenafil is a phosphodiesterase type 5 (PDE5) inhibitor that works by blocking the enzyme that breaks down cyclic GMP in smooth muscle tissue. Elevated cGMP levels cause smooth muscle relaxation and vasodilation, supporting vascular function. This is a compounded formulation.

PDE5 Inhibition Mechanism
PDE5 is expressed primarily in the smooth muscle of blood vessels and the corpus cavernosum. By inhibiting PDE5, sildenafil prevents the breakdown of cGMP — the second messenger that causes smooth muscle relaxation and vasodilation in response to nitric oxide signaling.
Onset and Duration
Standard sildenafil has an onset of approximately 30-60 minutes and duration of 4-6 hours. The ODT form may have faster onset due to sublingual absorption. Patients taking nitrates cannot use PDE5 inhibitors due to risk of severe hypotension.
Compounded vs. FDA Approved
Compounded sildenafil is not FDA-approved as safe and effective. FDA-approved versions of sildenafil exist as brand-name products. Compounded versions are prepared by licensed pharmacies for patients with specific needs.
Contraindications
PDE5 inhibitors are contraindicated with nitrate medications, certain alpha-blockers, and in patients with specific cardiovascular conditions. Physician evaluation is essential to identify contraindications before prescribing.
