About This Protocol

Injectable Methylcobalamin (Vitamin B-12) provides direct systemic delivery of the active methylated form of B-12. Unlike cyanocobalamin found in most oral supplements, methylcobalamin is the form that directly participates in methylation reactions without requiring hepatic conversion.

Methylcobalamin vs Cyanocobalamin
Methylcobalamin is the active coenzyme form of B-12, directly involved in methionine synthesis and myelin formation. Cyanocobalamin requires hepatic conversion to methylcobalamin before it can be utilized. Injectable methylcobalamin bypasses this conversion.
Neurological Function
B-12 is required for myelin sheath synthesis — the protective coating on nerve fibers. Deficiency is associated with peripheral neuropathy and cognitive changes. Injectable delivery achieves higher serum levels than oral supplementation.
Methylation Cofactor
Methylcobalamin is a cofactor for methionine synthase, which converts homocysteine to methionine. Elevated homocysteine is associated with cardiovascular risk. B-12 sufficiency supports homocysteine regulation.
Absorption Advantage
Injectable B-12 achieves serum levels substantially higher than oral supplementation, which is limited by intrinsic factor availability in the gut. This is particularly relevant for patients with GI absorption issues or pernicious anemia.
